Nasal Flu Vaccinations
On Thursday 28 and Friday 29 November 2024, the Gloucestershire Health and Care Immunisation Team will be attending Five Acres High School to offer students from all year groups the nasal flu vaccination
To provide consent for your child to receive the vaccination, please complete the form at this link - https://www.ghcimmunisations.co.uk/Forms/Flu and enter the school's unique code - GC144013
The team no longer uses paper consent forms, so please also complete the online form if you wish to decline the school-based session. The online portal will close 2 working days prior to the first session. A letter with further details has been emailed to parents/carers.
